Menggunakan PEMEtrexed bersama dengan indometasin dapat meningkatkan efek PEMEtrexed. Hubungi dokter anda jika anda mengalami demam, menggigil, nyeri tubuh, gejala flu, mulut luka atau bercak putih, dan diare yang parah. PEMEtrexed dapat menurunkan sel-sel darah yang membantu tubuh anda melawan infeksi. MENYESUAIKAN DOSIS INTERVAL: pemberian bersamaan dengan obat anti-inflamasi nonsteroid (Nsaid) dapat meningkatkan konsentrasi plasma dari pemetrexed. Mekanisme belum dijelaskan, tetapi mungkin terkait dengan NSAID penghambatan prostaglandin ginjal. Penggunaan Nsaid telah dikaitkan dengan nefropati dimanifestasikan sebagai peningkatan dalam serum kreatinin dan BUN, nekrosis tubular, glomerulitis, ginjal nekrosis papiler, nefritis interstitial akut, sindrom nefrotik, dan gagal ginjal. Sejak pemetrexed adalah terutama dieliminasi tidak berubah oleh ginjal ekskresi, pemberian bersamaan dengan Nsaid dapat mengakibatkan keterlambatan dan/atau penurunan clearance. Harian ibuprofen dosis 400 mg empat kali sehari telah terbukti mengurangi pemetrexed clearance sekitar 20% pada pasien dengan fungsi ginjal normal, sedangkan aspirin pada 325 mg setiap 6 jam tidak. Efek dari dosis yang lebih tinggi dari ibuprofen atau aspirin adalah tidak diketahui.

MANAJEMEN: Ibuprofen 400 mg dan aspirin pada 325 mg empat kali sehari, atau kurang, dapat digunakan dengan pemetrexed pada pasien dengan fungsi ginjal normal. Namun, disarankan untuk hati-hati pada pasien dengan ringan sampai sedang insufisiensi ginjal (bersihan kreatinin 45 sampai 79 mL/menit). Pasien-pasien ini harus menghindari mengambil Nsaid dengan singkat eliminasi setengah-hidup (misalnya, diklofenak, etodolac, fenoprofen, flurbiprofen, ibuprofen, indometasin, ketoprofen, ketorolac, meclofenamate, asam mefenamat, sulindac, tolmetin, dosis rendah salisilat) 2 hari sebelum dan 2 hari setelah pemetrexed administrasi. Jika pemberian bersamaan diperlukan, pasien harus dipantau secara ketat untuk toksisitas, terutama myelosupresi, nefrotoksisitas, dan toksisitas gastrointestinal. Dengan tidak adanya data mengenai penggunaan bersama Nsaid dengan panjang setengah-hidup, menahan NSAID dosis selama setidaknya 5 hari sebelum sampai 2 hari setelah pemetrexed administrasi dianjurkan.
